Privacy Statement

By means of this statement I want to let you know that I handle your personal data with care. I do not record more information than is necessary for contact and service. Collected personal data will not be sold or made available to third parties. Except for special circumstances, for example if this is legally required or necessary for the service.

If you visit my website www.coressence.nl, you will find various pages, perhaps make a request for information or an introduction via the contact form or via the mail. In doing so, you leave online traces and/or personal data.

Personal data is also shared when entering into a therapy process. This requires conscious and vigilant protection. As an entrepreneur I am obliged to inform you about how I handle your personal data. That is expressed in this Privacy Statement.

 

Careful and confidential

If you ask a question through the contact form, I use your email address or phone number to contact you. The personal explanation you may include will not be kept in my mailbox any longer than necessary. I periodically delete this data. The mailbox works with an encrypted (TLS) connection and is password protected. If you sign up for a possible future newsletter I will use your data only for that purpose. More about that below in detail.

 

What personal data are collected?

Because you visit the website, fill out a contact form, or subscribe to a newsletter, the following personal data are processed: first and last name, phone number, email address and data about your activities on the website. If you purchase services from me, such as therapy or group lessons, I will also store other personal data such as your address, date of birth, or that which is necessary for the counseling. More about that in the Therapy File.

 

Why are these requested?

I require these personal details to contact you and to be able to carry out the services (therapy, group therapy, coaching, workshop). I use your name and contact information to maintain contact and inform you about relevant issues related to the counseling. All data you share by email, I will store carefully. Your data may also be used to send newsletters (if you have subscribed to them) or otherwise inform you about my business activities, and to analyze visitor behavior to improve the website.

 

Your data will under no circumstances be sold to others.

 

Sharing data with third parties

Your personal data will only be shared with third parties if I am required to do so on grounds of a legal obligation or a legitimate interest such as reasonable business operations:

  • Website host, for storage of data from contact forms
  • Bank, for processing payments
  • Google Analytics, for the analysis of website visitor data
  • Bookkeeper/tax adviser, for access to invoices with address data
  • Newsletter software, for the digital newsletter (not yet on the agenda)
  • Website building
  • Newsletter software, for the digital newsletter (not yet on the agenda).

Processor agreements are/will be in place with these processors that include privacy terms.

At this moment, February 2021, I am not yet dealing with newsletter software.

This might change in the future, which is why I have included it as a possibility here.

 

Cookies

A cookie is a small and simple text file that is stored on your computer or mobile device when you first visit a website. On my website, only technical and functional cookies are used to make my website work properly and to ensure ease of use.

In addition, Google Analytics tracks general visitor data, but not your IP address.

Through the settings of your internet browser you can remove previously stored information (cookies).

 

Google Analytics in a privacy-friendly manner

Google Analytics is a web analytics tool that analyzes visit and click behavior on the website. On the websites of Google and Google Analytics you will find their privacy policies with more information. This data collection is done in a privacy friendly way; the unique customer part of the IP address is anonymized.

 

Your data is well protected

I take the protection of your data seriously. My computer is protected with appropriate and effective technical measures and the website uses a reliable HTTPS connection with Secure Socket Layer (SSL) certificate.

You can recognize  a HTTPS connection by the padlock and the word “secure” in front of the url. The provider of the practice software also ensures encryption of digital files, security of network connections via SSL technology and similar technology.

Therapy file

Mandatory in application of (psycho) therapy within the framework of the law WGBO.

For proper guidance it is necessary that I, as your supervising therapist, build a file. This is also a legal obligation, imposed by the Law on the Medical Treatment Agreement (WGBO). As a therapist I am affiliated with the professional organization FAGT and with RBCZ, and I adhere to the requirements and conditions they set for professional practice.

Your file contains your personal data, notes about your history and information about the coaching process. The file may also contain information that is necessary for your guidance and that I, with your explicit consent, have requested from another healthcare provider, such as your GP or a colleague.

 

Access to therapy file

I handle your personal and medical/psychic data with care and ensure that no unauthorized persons have access to these data. As your supervising therapist I have sole access to the data in the file. I have a legal duty of confidentiality (professional secrecy).

The data from your file can also be used:

  • To inform other health care providers, e.g. when the therapy is completed or in case of referral to another practitioner. This happens only with your explicit consent
  • For use in case of observation, during my (unplanned) absence.
  • For anonymized use during peer review.

If I want to use your data for any other reason, I will first inform you and ask your explicit permission.

This information in the client file will, as required by the WGBO, be stored for 20 years.

 

Privacy on the invoice

On the invoice you receive you will find the information required by the health insurer, so you can claim the bill from your health insurer. This includes:

  • Your name, address and place of residence
  • Date of birth
  • The date(s) of the session(s)
  • A short description, such as “Treatment energetic therapy”, with the corresponding performance code
  • The costs of the session
